Problem? form board moved

Two year old house. It appears the form board moved when the slab was poured. The brick is flush with slab at the back of the house and the ledge gets wider towards the front. About 2-3 inches. I've never seen this before and not sure how to report it and if this could be a potential problem later on. No evidence of any movement.

Foundation or maybe just the brick ledge poured out-of-square, the mason laid up the brick square and true.

As long as the through the wall flashing is installed, there should not be any problems.
