I wrote this up as:

Water supply to toilet should be cold, not hot.

Any suggestions appreciated.

I hope that's not all you said. How about "improper and unprofessional installation of water supply line to toilet in ______ bathroom - have serviced by a professional plumber and all repairs made as needed"
