Does this B vent require fire blocking at both penetrations into the ceiling and attic? Or simply around the first penetration. The pics are poor at best and the room is thunderdome.

From what I can tell from the photos, and your description "ceiling", the answer would be that both require proper fireblocking, and that does not mean insulation around it.

Good catch.

I just zoomed in on it and it is just hanging there by its wires, no box behind it.

But not to worry, there is a sprinkler head right behind it to put the fire out. :)

I thought that looked like a sprinkler head, how common is this?

Gunnar, good eye! I missed that.

Chris, all the larger homes in this area are required to be equipped with fire suppression systems. This place was around 8000 feet. Distance from the fire station, size, and location to wooded areas will determine whether or not a system is needed.
