Is there a minimum clearance requirement for above a glass cooktop range?

Had an over the range microwave / vent combo unit with only 14" from cooking surface to the bottom of the unit.

Also while on the subject, does anyone know what clearance from a cooking surface to the bottom of the cabinets is required?
