Concrete slab has horizontal crack

Hi all;

This concrete step (approx 3' x 3') has a horzontal crack, and minor displacement. The crack appears old, with (dust and cobwebs in it).
Does any one have any ideas what may have caused this, and is it of any concern?

Concrete was poured over an already problematic surface. Cracking is caused by movement underneath and will only progress with time.

I don't see a step. I see a trip hazzard. That is how I would report it.
