I realize that over the roof tops of London in Mary Poppins, you see thousands of chimneys next to each other, but....

How on earth do I write this up?

You should know that the masonry fireplace and chimney were original and on the wall that connected the garage to the living room (brick exposed into garage). The garage was converted into living space at some point and they built a brick hearth against the exposed brick of the fireplace/chimney. Upon the hearth, they set a wood burning stove and without bothering to put any angles in the flue, they went straight out to the roof.

Of course, Level II inspection.

Ask yourself a few questions here:
Is this chimney rated 2,100F and properly installed to the listed instructions?
Why is the chimney rusting on the side towards the masonry flues?
How do I resolve the separation requirements?
What about that roof penetration?
Are there combustibles sandwiched in the wall behind the stove?
What other clearances or requirements?
Why this shroud over the masonry chimney--hiding crown damage?
What effect does the heated flue gases from the fireplace or heater have on the cooling sysyem of the air cooled chimney?

I wrote up several of the items that you mentioned and recommended that the metal flue be removed from service.
