Got a friend that needs to reroof his house. He's concerned that the inspector will question him about the garage door he installed in the back of his garage to gain access to his back yard. He didn't have a permit for that install.

LMAO, That is a common worry among people who don't pull permits.
The inspector either notices it or he doesn't.
If he notices it he either chooses to limit his inspection to the scope of the permit or he decides to persue the violation.
If he decides to persue it "your friend" will need to get a permit and correct any code violations that may exist. To make it right.
Relax, in the whole scheme of things thats not that big of a deal.

Yes no big deal if it was done correctly, although last I knew the statute of limitations was one year in washingto state
