Arcing Condition

What do you suppose the cause of this arcing and damage is from? The dead-front cover was clean. There were no obvious modifications.

Looks like someone hit it with a neutral or other phase wire.

That's not good. (But you knew that.)

I'll bet it got their attention (and probably left a little brown stain in their drawers as well!)
