The 11 7/8 I joist is cut at the top about 3 inches wide and about 3 inches down into the joist. Is this a good place for a head out.

Can't cut the top of I-joists.

You need an experienced carpenter to go on the truss manufacturers web, look at the details. There are details of laminating ply wood with a compression space in between the upper and lower flange, then cutting the joist in the way of your trap(I ASSUME) and constructing a box header for the plumbing rough, please dont cut the top and be a ....

If it is a trap is going for a toilet or tub or even a kitchen sink in a island, chances are its next to a partition load of some sort, these are manufactured joist that lock in as a designed system, depending what your looking at, you can create some trouble down the road by thinking its a small piece.

The OP was talking about an I-joist, not a truss.

I do stand corrected, the I joist manufacturer shows the lamination and framing technique guidelines
