Do you have to have receptacles in a kitchen island?

Yes.

E3901.4 Countertop receptacles. In kitchens pantries, break
fast rooms, dining rooms and similar areas of dwelling units,
receptacle outlets for countertop spaces shall be installed in
accordance with Sections E3901.4.1 through E3901.4.5 (see
Figure E3901.4). Where a range, counter-mounted cooking
unit, or sink is installed in an island or peninsular countertop
and the width of the countertop behind the range, counter-mounted
cooking unit, or sink is less than 12 inches (305
mm), the range, counter-mounted cooking unit, or sink has
divided the countertop space into two separate countertop
spaces as defined in Section E3901.4.4. Each separate
countertop space shall comply with the applicable requirements
of this section.

E3901.4.2 Island countertop spaces. At least one receptacle
outlet shall be installed at each island countertop space with a
long dimension of 24 inches (610 mm) or greater and a short
dimension of 12 inches (305 mm) or greater.

If this "Island" is permanently attached to the floor ( yes )

If it's on casters even if they are hidden from view then The" Cart" does not.
