Is a dishwasher allowed to be on the same gfi protected circuit as the kitchen outlets?

I will assume you mean a fixed in place DW. If so the answer is no.

I think it depends on the age of the house.
At one time, I think it was allowed.

If its a countertop or portable appliance, yes.

Has anyone EVER seen one of those in someone's home? I've been in literally thousands of homes and I have not.

Has anyone EVER seen one of those in someone's home? I've been in literally thousands of homes and I have not.One of my wife's female friends has one. It is something you would use in a tiny apartment. Yes, I have seen one. No, I did not inspect the place and wouldn't want to. :D
