Anyone know what the standard is for raised fireboxes (from floor) specifically the size of the HE when there is a raised firebox?

Hearth extensions. Hearth extensions of approved factory-built fireplaces shall be installed in accordance with the listing of the fireplace. The hearth extension shall be readily distinguishable from the surrounding floor area.

Gas or solid fuel (wood)?
Either way, it is per the manufacturer's installation instructions, but a gas fireplace will not require a hearth extension - at least not the ones I've seen - provided the clearance distance above the combustible material is met. As I recall, some require 20" of clearance from the bottom of the fireplace to the floor for no hearth below it.
