Anyone ever see the complete PT cable end brace completely out of the foundation? Or (2) right beside each other?

Wondering how this could be repaired. If at all.

Any SE's in the room have an opinion?

That looks like the dead end of the cable.

My guess is that those two cables have been cut, which means repairing the cables where they were cut in the slab, and, chipping all the concrete out around these ends and turning these ends into the live tensioned ends as the live tensioned ends are (apparently, as you did not see them) still in the slab at the other end of those cables.

It is possible that those are the live tensioned ends, however, either way, they indicate that those cables have been cut and need repairing, and the repair would be the same regardless of which end blew out and which end stayed put. Normally, the dead ends stay put and the live tensioned ends blow out when a cable is cut. There are even times when both ends are live tensioned ends, one is tensioned first, then the other.
