Re: roof decking gap
Sloppy work but not really worth making a note of. I'm sure someone will disagree

Re: roof decking gap
The gap, in all likelihood is inconsequential.
There are multiple layers of roofing in the valley and they'll easily span that hole. It'd be nice to see some metal flashing through the hole though.

Re: roof decking gap
No real code violation there. Like Bill said...just sloppy work. Before plywood, builders used planking for roof decking often of varying widths and this created lots of gaps, sometimes large. As long as the roofer did his job correctly there should be no problems. Agreed...it would be nice to see some metal flashing showing through, but I personally would not include this one in the report as a problem.

If you could see metal (flashing), that would mean, there is not any roofing felt in that valley.

Re: roof decking gap
If I see something like that I will often mention it, even if I don't think it needs to be corrected. It lets people know that I was looking and in the attic. It can also head off questions later on when they do look in the attic for themselves.

Re: roof decking gap
It could be a problem if the next Home Inspector happens to be wearing high heels.
You just never know nowadays.
I came across a female inspector the other day and I sh*# you not, she had a granny kitchen apron on with some of her tools in the pockets. WTFudge?
It wouldn't have been so bad, but the bunnies and chickens on the apron was a bit much.

Something I just noticed is that the OSB is not run perpendicular to the joist.

Wookalars are demonic Creatures of the Dark.
They will take out a HI in no time. Be on the look out for them.
