I may be that i am missing the obvious but how did they drive staples thru the wire and then into the foil?

I take it that they were using the staples to install the foil. Driving staples through the foil and hitting an energized wire resulting in the death.
The likely have wire strung haphazardly through the attic with no nail protection like it is here.
Seems that it would be the installers fault though, not really the material. It does point out the dangers of electrically conductive materials installed near electrical.
