Re: What do you see
Without seeing the door headjamb I can't comment on that. It appears to be installed incorerectly, but I just can't say. Otherwise I see:
(1) The obvious damaged siding.
(2) The siding is not spaced 1/8" at the doorjamb and caulked.
(3) The siding butt joint is not caulked.
(4) The door trim miter joint is not caulked.
(5) The nail heads are not flush with the siding surface.
That's about it.

Re: What do you see
Looks like it does not have a 1/4" gap above the head flashing either.
Nails may be either "snug" to the surface, not "flush" with the surface. If "flush", then countersink nail, caulk hole, add new nail "snug" to surface.
That's a pi$$ poor place to have a butt joint - right above and in line with the side of the trim. Place the joint off to one side and there will be less likelihood of water intrusion.
Also, shouldn't that flashing extend over past the top corner of the trim some?

EC Jerry:
That's a KB expansion joint.

Re: What do you see
If face nailed the nail should not be driven through the board it laps over.
