    What are your thoughts on this. I see it quite often.

    Maybe I'm missing it but I don't see any frayed cable.
    If this is the main service equipment then it is missing the bond screw and if it's not then those grounds need to be removed from the neutral bar.
    Plus the #4 copper grounds, look like they are attached to the can with self tapping screws ??? If so thats wrong
