Is there a grandfathering law that would allow this to be OK. House is 2001. Neutrals & grounds under the same lug.

Nope ... not other than the grandfather who did it was so old and had not been keeping up with the codes for so long that they were not yet aware of those new fangled Safe-T-Fuse fuses and their adapters.
Okay, sort of a yes and no, with the 'yes' being not quite allowed.
The 'yes' is that before the 2002 NEC, the code *did not specifically state* that only one neutral conductor per terminal was allowed. Note the emphasis on *specifically state*. In the 2002 NEC the code *did specifically state* that is not allowed.
The 'no' is because the code has specifically stated that the panels had to be installed in accordance with their listing and labeling, and as such, and because no panel was listed or labeled for more than one neutral conductor in a terminal, the answer is 'no'.
Was putting two neutral conductors in one terminal a "code violation" prior to 2002? No - but ... doing that would create an installation which was *not in accordance with* its listing and labeling, and THAT was a "code violation" prior to 2002, it still is now, and it had been for decades.
