Default Service entry neutral was 1/0 , 1 Hot was 3/0 other 2/0

Here we go again, same kinda question as last month. This one has 2/0 hot., 3/0 hot and a 1/0 neutral (as so appears to be). I need to go to Lowe's and get some wiring sizes to keep around because you cant read the writing on the cables. House was a 200 amp copper service single family dwelling built in 1995. How often is the NEC updated, what year code should this home fall under? Thanks Again

What was also puzzling is the fact neither bathrooms had GFCI receptacles, 1 outside and kitchen? (1995 house)

No tested every bathroom outlet, none of them tripped.
