Generator lock out switch.
I saw this lock out switch for generator back feed connection.
Are these code approved?

Looks like a factory breaker interlock. Was the breaker retention device in place on the generator breaker?
All answers based on unamended National Electrical codes.

Maybe I'm not seeing the photo right, but I don't see where that prevents both being on at the same time.
Am I just not seeing it?

I went back and zoomed in on the photo on my computer (I was looking on my phone earlier) and then I could see where raising the lockout up is blocked by the breaker when the main utility power breaker is 'on', and when the main utility power breaker is turned off - the lockout can slide up, which allows the generator breaker to be turned 'on'.
Hope those screws on the left are not very long ...

Normally just long enough to get a nut on the inside to hold the bracket.
