Not grounded and it will only work when the wall switch is on.

Flag it! That grounding adapter means the opener is not grounded! Seems the lack of a ground killed a kid awhile back who was in a water puddle outside the door when he touched it. The incident I recall was from the entire garage not having a ground and a missing GFCI, but the result is the same, DEAD PEOPLE!
