Default Distance from grade to plaster or screed on exterior of home in San Diego, CA

How many inches does the plaster screed or plaster if no screed have to be from grade on the exterior of a home in Carlsbad, CA? 2", 4" or what is the correct distance?

Minimum 2" above finished grade paved areas and 4" above earth. Invariably a dilemma occurs when a 3"-4" patio slab is poured or when pavers are installed on top of the previously minimally finished earth grade, abutting the exterior finish. Rarely is the ground regraded to accommodate the slab or paver thickness.
