I suggest you post this over in Chimneys, Fireplaces, etc. area and/or PM/email out to Bob Harper for verification...

I think that's a drainage system for a wet chimney without a cleanout door, perhaps orignally served a coal burning furnace or an incinerator (burning garbage).

If you don't hear back from Bob Harper, Dale, or others, and in the meantime anyway strongly recommend a complete inspection/analysis of the entirety of the chimney by a professional.

Is this chimney being used for venting gas burning appliances presently? Is this in the basement?

First of all thank you so much for the info. I will post in the right forum. It's not currently being used for any venting. I did recommend that a certify chimney company inspect the easy clean drain. Again Thank you

Very common, used to see them all the time in the City. Pretty sure we even had one in our old house. Chimney clean out and way for rain or condensation water to get out of the chimney instead of sitting in there and causing damage. Looks much better than mortar erosion and white powder around the bottom of the wall.
Leave it, take it out, depends on whether the chimney is active, sealed or not.
