Looks like it is mortared into the old cleanout connection. Connection is designed to fail over time. Yes you could install a neoprene hub but a new cleanout needs to be added at the PVC pipe for servicing.

And a sanitary tee is required there for the PVC. A clean-out access *is not* suitable for that use.
Obviously ... it "can" be done because you took a photo of it having been done, however, it is "not allowed" to be done - big different in those two words 'can' and 'allowed'.

The part about cement mortar joints has to do with clay sewer repairs. In the past clay sewers were all installed using oakum and cement.
