Default Double Tap At Main Lug

Looked in a ITE panel today and found a double tap @ the main lug, but other end went to a ITE Safety Switch box. Would this be acceptable

No, that lug is good for only one conductor.

Don't forget all those neutrals and grounds in the same terminals with each - that is not allowed either.

    To add

Appears there is no identified neutral ( not a big deal but just pointing it out)

You probably mentioned it in your report but they could use a few lessons on properly fastening those nm cables in the photo of the safety switch.

Often it's ok to have more than one equipment grounding conductor at the same terminal.
