Default Found a piece of carboard inside panel

Hello all. I found this on my employer's property this spring, by accident. Obviously, this is ill advised and has of course been removed. I didn't see any scorching or indeed anything else seriously wrong with this panel.

My question is, has anyone seen this before? Is there any conceivable reason why someone would want to do this? Just how dangerous is this? I can only imagine ignorance or sabotage came into play here.

Also noted that the backfed main breaker doesn't have a secondary hold down.

Thank you all for your feedback.

Probably placed there as a guard against accidental contact with the hot lugs.

All answers based on unamended National Electrical codes.
